What is Amondys 45?

Amondys 45 is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Sarepta Therapeutics, Inc. It is used for the treatment of Duchenne muscular dystrophy (DMD) in patients who have a confirmed mutation of the DMD gene that is receptive to exon 45 skipping. FDA approval of Amondys is under accelerated approval based on an increase in dystrophin production in skeletal muscle seen in patients treated with Amondys 45. Continued approval for this use may depend upon verification of a clinical benefit in confirmatory trials.

Duchenne is caused by a genetic mutation, or a change, in the dystrophin gene, where one or multiple exons are missing. This causes errors in the production of dystrophin protein, protein muscles need to function correctly.

Amondys 45 is an exon-skipping therapy. The goal of exon skipping is to allow the body to make a shorter form of the dystrophin protein. Amondys 45 is the third Sarepta RNA exon-skipping therapy for DMD patients that the FDA has approved. The other two are Exondys 51 (eteplirsen), for patients receptive to exon 51 skipping, and Vyondys 53 (golodirsen), for those with mutations receptive to skipping exon 53.

What doses of Amondys 45 are available?

Amondys 45 is available in injection form, in the following doses: 100 mg/2 mL in a single-dose vial.

Serum cystatin C, urine dipstick, and urine protein-to-creatinine ratio should be measured before starting treatment. The recommended dose is 30 milligrams per kilogram of body weight once weekly as an intravenous infusion over 35 to 60 minutes via an in-line 0.2-micron filter.

The active ingredient of Amondys 45 is casimersen, an antisense oligonucleotide of the phosphorodiamidate morpholino oligomer (PMO) subclass.
