What is Arikayce?

Arikayce is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Insmed Incorporated. Arikayce is an aminoglycoside antibacterial used in adults who have limited or no alternative treatment options, for the treatment of Mycobacterium avium complex (MAC) lung disease as part of a combination antibacterial drug regimen in patients who do not achieve negative sputum cultures after a minimum of 6 consecutive months of a multidrug background regimen therapy. Arikayce is for use in adults who have limited or no alternative treatment options. This drug is indicated for use in a specific limited population only. Arikayce is approved under accelerated approval based on achieving sputum culture conversion by month 6.

Limitation of Use – Arikayce has only been studied in patients with refractory MAC lung disease, defined as patients who did not achieve negative sputum cultures after a minimum of 6 consecutive months of a multidrug background regimen therapy. The use of Arikayce is not recommended for patients with non-refractory MAC lung disease.

What doses of Arikayce are available?

Arikayce is available in sterile, aqueous, liposome suspension form, in the following doses: 590 mg/8.4 mL.

Use Arikayce vials only with the Lamira nebulizer system. The recommended dosage in adults is once-daily oral inhalation of the contents of one 590 mg/8.4 mL vial.

Arikayce active ingredients

The active ingredient in Arikayce is amikacin sulfate.

Arikayce side effects

The most common side effects of Arikayce in clinical trials include:

  • Dysphonia
  • Chest discomfort, shortness of breath, bronchospasm
  • Upper airway irritation
  • Musculoskeletal pain, tiredness
  • Nausea and diarrhea

In rare instances, Arikayce can cause more serious side effects. These can include:

  • Respiratory adverse reactions – hypersensitivity pneumonitis, hemoptysis, bronchospasm, and exacerbation of underlying pulmonary disease that have led to hospitalizations
  • Worsening of chronic obstructive pulmonary disease (COPD)
  • Ototoxicity – monitor closely if you have known or suspected auditory or vestibular dysfunction. Tinnitus may be an early symptom of cytotoxicity
  • Nephrotoxicity
  • Neuromuscular blockade – aminoglycosides may worsen muscle weakness
  • Embryo-Fetal toxicity – can cause total, irreversible, bilateral congenital deafness in pediatric patients

Arikayce drug interactions

Arikayce can interact with other medications. These include:

  • Drugs with neurotoxic, nephrotoxic, or ototoxic potential
  • Ethacrynic acid, furosemide, urea, or mannitol – some diuretics can enhance aminoglycoside toxicity by altering aminoglycoside concentrations in serum and tissue

This list is not exhaustive and other prescription drugs may interact with Arikayce. Tell your healthcare provider about all the medicines you take, including prescription medicines and over-the-counter medicines, vitamins, and herbal supplements.

Arikayce contraindications

You should not use Arikayce if you:

  • Are allergic to the active ingredient amikacin or similar antibiotics
  • Are allergic to any of the other ingredients in Arikayce
  • Are pregnant or are planning to become pregnant

Talk to your doctor before using Arikayce if you:

  • Are taking any of the medications that could interact with Arikayce
  • Have asthma, COPD, wheezing, or other breathing problems
  • Have hearing problems
  • Have vertigo
  • Have renal impairment
  • Have a nerve-muscle disorder, such as myasthenia gravis
  • Are breastfeeding or are planning to breastfeed

Arikayce generic

Arikayce is the trademark brand name for amikacin liposome inhalation suspension manufactured by Insmed, Inc. A generic version of amikacin liposome inhalation suspension is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Arikayce savings through NiceRx.
