How do I apply for the Camzyos patient assistance program?

We make it as easy as possible to apply for the Camzyos patient assistance program. The first step is to complete our enrollment application. Tell us about any medications you are taking, including Camzyos. We will also ask for details about your healthcare provider, insurance, and your household income. This information is required by the pharmaceutical manufacturers who ship your medication.

If we determine that you may be eligible for Camzyos assistance, we will handle the application process for you. We will work directly with your healthcare provider to prepare your application documents. Once we have received the signed application and supporting documents, we will submit them to the patient assistance program on your behalf. Our service continues once you are enrolled in the Camzyos patient assistance program, and we will automatically request your Camzyos medication refills for up to 1 year.

What is Camzyos?

Camzyos is a cardiac myosin inhibitor manufactured by Bristol Myers Squibb. Camzyos gained FDA approval (U.S. Food and Drug Administration) based on data from Myokardia’s phase 3 Explorer-HCM trial to treat adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM), a cardiovascular disease. Traditionally, the treatment of HCM patients has been based on managing the symptoms with drugs such as beta-blockers or calcium channel blockers.

Camzyos is only available through a restricted program called the Camzyos Risk Evaluation and Mitigation Strategy Program and can only be dispensed by certified pharmacies that participate in the Camzyos REMS Program.

What doses of Camzyos are available?

Camzyos is available in capsule form, in the following doses: 2.5 mg, 5 mg, 10 mg, and 15 mg capsules.

Please see the full prescribing information including the boxed warning and medication guide for the risk of heart failure. Camzyos reduces left ventricular ejection fraction (LVEF) and can cause heart failure due to systolic dysfunction. Echocardiogram assessments of LVEF are needed before and during treatment.
