What is Carbaglu?

Carbaglu (carglumic acid) tablets for oral suspension are an FDA (U.S. Food and Drug Administration) approved medication distributed by Recordati Rare Diseases Inc. Carbaglu is a carbamoyl phosphate synthetase 1 (CPS 1) activator indicated in pediatric and adult patients for:

  • Adjunctive therapy for the treatment of acute hyperammonemia due to the deficiency of the liver enzyme N-acetylglutamate synthase (NAGS)
  • Maintenance therapy for the treatment of chronic hyperammonemia due to the deficiency of the hepatic enzyme N-acetylglutamate synthase (NAGS)
  • Propionic acidemia (PA)
  • Methylmalonic acidemia (MMA)

People with NAGS deficiency, propionic academia, or methylmalonic acidemia may experience acute high blood ammonia levels. At high levels, ammonia is toxic, especially to the brain. Lowering high blood ammonia levels quickly is essential.

What doses of Carbaglu are available?

Carbaglu is available in a tablet dosage form, in the following doses: 200 mg.

Carbaglu treatment should be initiated by a physician experienced in metabolic disorders The recommended initial dose range for acute hyperammonemia is 100 mg/kg/day to 250 mg/kg/day. Adjust the dose to maintain normal plasma ammonia levels based on age. Divide the total daily dose into two to four doses to be given immediately before meals or feedings. Each 200 mg tablet should be dispersed in a minimum of 2.5 ml of water and taken immediately. Carbaglu can be taken orally or through a nasogastric tube. Carbaglu tablets should not be swallowed whole or crushed.

Discard Carbaglu tablets one month after first opening the container. Read the labeling instructions for use, and always speak with a healthcare professional about any changes to your dose so they can monitor and evaluate your condition.

Carbaglu active ingredients

The active ingredient of Carbaglu is carglumic acid. It belongs to a class of drugs called Urea cycle disorder treatment agents.

Carbaglu side effects

The most common side effects of Carbaglu in clinical trials include:

  • Infections, ear infection
  • Vomiting, abdominal pain, diarrhea
  • Pyrexia
  • Tonsilitis
  • Anemiaiarrhea
  • Nasopharyngitis
  • Dysgeusia
  • Asthenia
  • Hyperhidrosis
  • Influenza
  • Weight decreased
  • Somnolence
  • Rash

In rare instances, Carbaglu can cause more serious side effects. These can include:

  • Hyperammonemia – in the initial treatment of NAGS deficiency, protein restriction is recommended. When plasma ammonia level is normalized, dietary protein intake can usually be reintroduced
  • Hemoglobin decreased
  • Pneumonia
  • Anorexia

Carbaglu drug interactions

Other drugs may interact with Carbaglu, including prescription drugs and over-the-counter medicines, vitamins, and herbal products. Tell your health care provider about all of your medicines including any supplements you are taking.

Carbaglu contraindications

You should not use Carbaglu if you:

  • Are allergic to the active ingredient carglumic acid
  • Are allergic to any of the other ingredients in Carbaglu
  • Are pregnant or are planning to become pregnant – pregnant women are advised to be cautious when taking Carbaglu

Talk to your doctor before using Carbaglu if you:

  • Are taking any of the medications that could interact with Carbaglu
  • Are breastfeeding or are planning to breastfeed – human milk-feeding is not recommended

Carbaglu is the trademark brand name for carglumic acid distributed by Recordati Rare Diseases Inc. A generic version of carglumic acid is available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Carbaglu savings through NiceRx.
