What is Envarsus XR?

Envarsus XR is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Veloxis Pharmaceuticals. It is a calcineurin-inhibitor immunosuppressant indicated for:

  • The prophylaxis of organ rejection in de novo kidney transplant patients post-transplant in combination with other immunosuppressants
  • The prophylaxis of organ rejection in kidney transplant recipients converted from tacrolimus immediate-release formulations in combination with other immunosuppressants

There is an increased risk for developing serious infections, lymphoma, and malignancies with Envarsus XR or other immunosuppressants that may lead to hospitalization or death.

What doses of Envarsus XR are available?

Envarsus XR is available in extended-release tablet form, in the following doses: 0.75 mg, 1 mg, 4 mg.

Take Envarsus XR once daily on an empty stomach at the same time of day, preferably in the morning. Avoid eating grapefruit or drinking grapefruit juice or alcohol. African-American patients may need to be titrated to higher dosages to achieve the target tacrolimus concentrations. Patients with severe hepatic impairment may require a lower starting dose. Frequent monitoring of trough concentrations is recommended.

If you miss your dose of Envarsus XR, take it as soon as possible, but no longer than 15 hours after missing your dose. If the time after you miss your dose is more than 15 hours, the missed dose should be skipped and the next dose should be taken the following morning at your normal scheduled time. Do not take 2 doses at the same time.

Envarsus XR active ingredients

Envarsus XR contains tacrolimus as the active ingredient.

Envarsus XR side effects

The most common side effects of Envarsus XR in clinical trials (incidence ≥15%) include:

  • Diarrhea, constipation, headache
  • Anemia
  • Urinary tract infection
  • Diabetes mellitus
  • Peripheral edema
  • Hyperkalemia
  • For kidney transplant patients converting from immediate-release to extended-release tacrolimus, the most common adverse reactions include diarrhea and blood creatinine increased

In rare instances, Envarsus XR can cause more serious side effects. These can include:

  • Progressive multifocal leukoencephalopathy (PML)
  • New-Onset diabetes after transplant
  • Nephrotoxicity – monitor renal function
  • Neurotoxicity – including the risk of posterior reversible encephalopathy syndrome (PRES)
  • Hyperkalemia – monitor serum potassium levels
  • Hypertension (high blood pressure)
  • QT prolongation
  • Pure red cell aplasia

Envarsus XR drug interactions

Envarsus XR can interact with other medications. These include:

  • Mild or moderate CYP3A inhibitors – erythromycin
  • Calcium channel blockers – verapamil, diltiazem, nifedipine, nicardipine
  • Azole antifungals – clotrimazole, fluconazole, voriconazole
  • Aluminum, magnesium antacids
  • Cyclosporine, sirolimus, temsirolimus, ziprasidone
  • Other drugs that may increase the level of potassium in the blood – amiloride, spironolactone
  • Drugs that weaken the immune system and increase the risk of infection – natalizumab, rituximab
  • St. John’s wort
  • HIV and HCV protease inhibitors – nelfinavir, ritonavir, boceprevir, telaprevir
  • Rifamycins – rifampin, rifabutin
  • Avoid live vaccines

This list is not exhaustive and other prescription drugs may interact with Envarsus XR. Tell your healthcare provider about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements.

Envarsus XR contraindications

You should not use Envarsus XR if you:

  • Are allergic to the active ingredient tacrolimus
  • Are allergic to any of the other ingredients in Envarsus XR
  • Are pregnant or are planning to become pregnant

Talk to your doctor before using Envarsus XR if you:

  • Are taking any of the medications that could interact with Envarsus XR
  • Are breastfeeding or are planning to breastfeed

Envarsus XR generic

Envarsus XR is the trademark brand name for tacrolimus manufactured by Veloxis Pharmaceuticals. A generic version of tacrolimus is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Envarsus XR savings through NiceRx.
