What is Hycamtin?
Hycamtin is an FDA (U.S. Food and Drug Administration) approved medication manufactured by GlaxoSmithKline. It is classed as an anticancer antineoplastics topoisomerase i inhibitor. Camptothecan analogs are derived from the Asian “Happy Tree” (Camptotheca acuminata).
Topoisomerase inhibitors are drugs that interfere with the action of topoisomerase enzymes (topoisomerase I and II). Topoisomerase enzymes control the manipulation of the structure of DNA necessary for cell division and replication.
Hycamtin is indicated for the treatment of:
- Patients with metastatic ovarian cancer (carcinoma) after disease progression on or after initial or subsequent chemotherapy, as a single agent
- Patients with small-cell lung cancer (SCLC) platinum-sensitive disease who progressed at least 60 days after initiation of first-line chemotherapy, as a single agent. (Topotecan is often given in combination with paclitaxel as a first-line treatment for extensive-stage small-cell lung cancer)
- Patients with Stage IV-B, recurrent, or persistent cervical cancer which is not amenable to curative treatment, in combination with cisplatin
What doses of Hycamtin are available?
Hycamtin is available in injection form, in the following doses: 4 mg (free base) lyophilized powder in a single-dose vial.
Ovarian cancer and Small cell lung cancer – 1.5 mg/m2 by intravenous infusion over 30 minutes daily for 5 consecutive days, starting on Day 1 of a 21-day cycle.
Cervical cancer – 0.75 mg/m2 by intravenous infusion over 30 minutes on Days 1, 2, and 3, with cisplatin 50 mg/m2 on Day 1, of a 21-day cycle.
Reduce the dose if creatinine clearance (CLcr) is 20 to 39 mL/min due to renal impairment

Hycamtin active ingredients
Hycamtin contains topotecan hydrochloride as the active ingredient. Topoisomerase I induces reversible single-strand breaks in DNA. Topotecan’s mechanism of action is to bind to the topoisomerase I-DNA complex and prevents re-ligation of these single-strand breaks. The cytotoxicity of topotecan is thought to be due to double-strand DNA damage produced during DNA synthesis when replication enzymes interact with the complex formed by topotecan, topoisomerase I, and DNA.
Hycamtin side effects
The most common side effects of Hycamtin in clinical trials include:
- Administration site conditions
- Decreased bone marrow function. This effect may lead to a low number of blood cells such as red cells, white cells, and platelets
- Neutropenia
- Anemia (reduced red blood cell count)
- Thrombocytopenia (reduced platelet count)
- Febrile neutropenia (reduced white blood cells)
- Nausea, vomiting, tiredness, diarrhea
- Asthenia, dyspnea, pneumonia, abdominal pain
- Pain, vomiting, and infection
- Feeling generally weak and tired
- Weight loss and loss of appetite (anorexia)
- Constipation
- Burning sensation when urinating, which may be caused by a urinary infection
- Fever
- Alopecia (hair loss)
- Bone pain
- Mouth sores
- Rash
- Shortness of breath
- Cough
- Headache
In rare instances, Hycamtin can cause more serious side effects. These can include:
- Interstitial lung disease (ILD)
- Extravasation and tissue injury
- Embryo-fetal toxicity – use effective contraception during treatment

Hycamtin contraindications
You should not use Hycamtin if you:
- Are allergic to the active ingredient topotecan hydrochloride
- Are allergic to any of the other ingredients in Hycamtin
- Have a history of severe hypersensitivity reactions to topotecan
- Are breastfeeding or are planning to breastfeed
- Are pregnant or are planning to become pregnant
