What is Istodax?

Istodax is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Celgene Corporation. It is a histone deacetylase (HDAC) inhibitor indicated for the treatment of cutaneous T-cell lymphoma (CTCL) and peripheral T-cell lymphoma (PTCL) in patients who have received at least one prior systemic therapy.

What doses of Istodax are available?

Istodax is available in injection form, in the following doses: 10 mg, supplied with one diluent vial containing 2 mL of solution.

14 mg/m2 is administered intravenously (IV) over a 4-hour period on days 1, 8, and 15 of a 28-day cycle. Repeat cycles every 28 days. Treatment discontinuation or interruption with or without dose reduction to 10 mg/m2 may be needed to manage adverse drug reactions. Continue treatment until disease progression or intolerable toxicity.

Istodax active ingredients

The active ingredient of Istodax is romidepsin. It belongs to a class of drugs called histone deacetylase inhibitors (HDAC). HDACs are enzymes that control histones, which are important proteins in the formulation of DNA. Istodax blocks HDACs so histones cannot be used properly. This interferes with the genetic makeup of cancer cells, leading to cell death.

Istodax side effects

The most common side effects of Istodax in clinical trials include:

  • Nausea, vomiting
  • Tiredness
  • Infections
  • Anorexia
  • Loss of appetite
  • Changes in sense of taste
  • Constipation
  • Itching
  • Anemia (low red blood cells)
  • Thrombocytopenia
  • Neutropenia (low white blood count)
  • Leukopenia
  • Lymphopenia
  • Electrocardiographic (ECG) T-wave changes

In rare instances, Istodax can cause more serious side effects. These can include:

  • Reactivation of hepatitis B virus infection
  • QT prolongation – ensure that potassium and magnesium are within the normal range before administration of Istodax
  • Serious infections – symptoms include fever, cough, shortness of breath with or without chest pain, burning with urination, flu-like symptoms, muscle aches
  • Tumor Lysis Syndrome (TLS) – rapid breakdown of cancer cells

Istodax drug interactions

Istodax can interact with other medications. These include:

  • Coumadin derivatives – warfarin
  • Strong CYP3A4 inhibitors
  • Potent CYP3A4 inducers
  • Azole antifungals – itraconazole, and ketoconazole
  • Rifamycins such as rifabutin, rifampin
  • St. John’s wort
  • Drugs used to treat seizures such as carbamazepine
  • Hormonal birth control such as pills, patches, or rings

This list is not exhaustive and other prescription drugs may interact with Istodax.

Istodax contraindications

You should not use Istodax if you:

  • Are allergic to the active ingredient romidepsin
  • Are allergic to any of the other ingredients in Istodax
  • Are pregnant or are planning to become pregnant – Embryo-fetal toxicity may occur and harm your unborn baby. Females of reproductive potential are advised to use effective contraception during treatment and for 1 month after the last dose

Talk to your doctor before using Istodax if you:

  • Are taking any of the medications that could interact with Istodax
  • Have heart problems or a heart rhythm disorder
  • Have long QT syndrome (in you or a family member)
  • Have kidney disease
  • Have hepatic impairment (especially hepatitis B)
  • Have an electrolyte imbalance
  • Are breastfeeding or are planning to breastfeed

Istodax generic

Istodax is the trademark brand name for romidepsin manufactured by Celgene Corporation. A generic version of romidepsin is available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Istodax savings through NiceRx.
