What is Livmarli?

Livmarli is an FDA (U.S. Food and Drug Administration) approved medication manufactured for Mirum Pharmaceuticals, Inc. Livmarli is an ileal bile acid transporter (IBAT) inhibitor with FDA approval for the treatment of cholestatic pruritus (itching associated with liver disease) in patients with Alagille syndrome (ALGS) and cholestasis 1 year of age and older. ALGS is a rare genetic disorder caused by abnormalities in bile ducts that can lead to progressive liver disease. Livmarli is not recommended for use in pediatric patients less than 1 year of age.

What doses of Livmarli are available?

Livmarli is available in oral solution form, in the following doses: 9.5 mg/mL.

The recommended daily dose is 380 mcg/kg once daily, 30 minutes before the first meal of the day. The starting dose is 190 mcg/kg orally once daily, increased to 380 mcg/kg once daily after one week, as tolerated.

Livmarli active ingredients

Livmarli contains maralixibat as the active ingredient. It is an ileal bile acid transporter, IBAT inhibitor that reduces serum bile acid levels in the body by interrupting the recirculation of bile acids to the liver and increasing their excretion in feces.

Livmarli side effects

The most common side effects of Livmarli in clinical trials compared to placebo include:

  • Diarrhea, abdominal pain, vomiting
  • Fat-soluble vitamin deficiency (FSV) – supplement if deficiency is observed. If deficiency persists or worsens consider discontinuing treatment
  • Liver test abnormalities – obtain baseline liver tests and monitor during treatment
  • Gastrointestinal bleeding
  • Bone fractures

In rare instances, Livmarli can cause more serious side effects. These can include:

  • Portal hypertension
  • Hepatic decompensation event

Livmarli drug interactions

Livmarli can interact with other medications. These include:

  • Bile acid-binding resins – cholestyramine, colesevelam, or colestipol
  • Statins

Livmarli contraindications

You should not use Livmarli if you:

  • Are allergic to the active ingredient maralixibat
  • Are allergic to any of the other ingredients in Livmarli

  • Have liver problems
  • Are pregnant or are planning to become pregnant
  • Are breastfeeding or are planning to breastfeed

Livmarli generic

Livmarli is the trademark brand name for maralixibat manufactured for Mirum Pharmaceuticals, Inc. A generic version of maralixibat is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Livmarli savings through NiceRx.
