What is Orenitram?

Orenitram is an FDA (U.S. Food and Drug Administration) approved medication manufactured by United Therapeutics Corporation. Orenitram is a prostacyclin vasodilator indicated for the treatment of pulmonary arterial hypertension (common symptoms of PAH include tiredness and shortness of breath or difficulty breathing, especially when doing daily activities) (WHO Group 1) to improve exercise capacity.

The study that established effectiveness included predominately patients with WHO (World Health Organization) functional class II-III symptoms and etiologies of idiopathic or heritable PAH (75%) or PAH associated with connective tissue disease (19%). As the sole vasodilator, the effect on exercise is small. Orenitram has not been shown to add to other vasodilator therapy.

What doses of Orenitram are available?

Orenitram is available in extended-release tablet form, in the following doses: 0.125 mg, 0.25 mg, 1 mg, and 2.5 mg.

The recommended starting dose is 0.25 mg orally every 12 hours with food. The total daily dose can be divided and given every 8 hours with food, in titrations of 0.125 mg 3 times daily.
The maximum dose will depend on tolerability. The maximum dose studied was 21 mg orally every 12 hours with food.

Do not crush, chew, or break an extended-release tablet. Swallow the tablet whole. Orenitram is made with a shell that is not absorbed in the body and part of the tablet shell may appear in your stool. Call your doctor if you have new or worsening PAH symptoms such as shortness of breath (even with mild exertion), tiredness, chest pain, and pale skin.

Store the tablets at room temperature away from moisture and heat. Read the full patient information and the additional information supplied with your medication. Always speak with your healthcare provider for medical advice or about any changes to your dose so they can monitor and evaluate your condition.

Orenitram active ingredients

Orenitram contains treprostinil as the active ingredient.

Orenitram side effects

The most common side effects of Orenitram in clinical trials compared to placebo include:

  • Headache
  • Nausea
  • Diarrhea

In rare instances, Orenitram can cause more serious adverse effects. These can include:

  • Abrupt discontinuation of Orenitram may cause worsening of your PAH symptoms
  • Increased risk of bleeding, particularly if you are receiving anticoagulants
  • In patients with diverticulosis, Orenitram tablets can become lodged in a diverticulum

Orenitram drug interactions

Orenitram can interact with other medications. These include:

  • Blood pressure-lowering drugs, such as diuretics, antihypertensive agents, or vasodilators
  • Strong CYP2C8 inhibitors
  • Do not take Orenitram with alcohol

This list is not exhaustive and other prescription drugs may interact with Orenitram.

Orenitram contraindications

You should not use Orenitram if you:

  • Are allergic to the active ingredient treprostinil
  • Are allergic to any of the other ingredients in Orenitram
  • Have severe hepatic impairment (Child-Pugh Class C)
  • Are pregnant or are planning to become pregnant – animal reproductive studies have shown an adverse effect on the fetus

Talk to your doctor about your medical condition before using Orenitram if you:

  • Are taking any of the medications that could interact with Orenitram
  • Have diverticulosis
  • Take another medicine that contains treprostinil, such as Remodulin, or Tyvaso
  • Are breastfeeding or are planning to breastfeed – it is not known if Orenitram passes into your breast milk

Orenitram generic

Orenitram is the trademark brand name for treprostinil manufactured by United Therapeutics Corporation. A generic version of treprostinil is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Orenitram savings through NiceRx.
