If you are eligible for the Palynziq patient assistance program you can receive your medication for free. Our monthly service fee of $49 per medication covers the cost of our services in applying for the program and requesting your ongoing refills on your behalf. What is Palynziq?

Palynziq is an FDA (U.S. Food and Drug Administration) approved medication manufactured by BioMarin Pharmaceutical Inc. Palynziq is a phenylalanine-metabolizing enzyme used to reduce blood phenylalanine concentrations in adult patients with phenylketonuria (PKU), who have uncontrolled blood phenylalanine concentrations greater than 600 micromol/L on existing management.

Palynziq is a novel enzyme substitution therapy that helps meet an unmet need in PKU patients who have been unable to control their blood Phe levels with current treatment options. Patients with PKU are unable to break down phenylalanine (Phe), an amino acid present in protein-containing foods and high-intensity sweeteners used in a range of foods and drinks.

Risk of Anaphylaxis – see the full prescribing information for complete boxed warning. Anaphylaxis has been reported after administration of Palynziq and may occur at any time during treatment. Administer the initial dose of Palynziq under the supervision of a healthcare provider equipped to manage anaphylaxis, and closely observe patients for at least 60 minutes following injection. Prior to self-injection, confirm patient competency with self-administration, and patient’s and observer’s ability to recognize signs and symptoms of anaphylaxis and to administer auto-injectable epinephrine, if needed. Prescribe auto-injectable epinephrine. Prior to the first dose, instruct the patient and observer on its appropriate use. Instruct the patient to seek immediate medical care upon its use. Instruct patients to carry auto-injectable epinephrine with them at all times during treatment. Palynziq is available only through a restricted program called the Palynziq REMS Program.

What doses of Palynziq are available?

Palynziq is available in injection form, in the following doses: 2.5 mg/0.5 mL, 10 mg/0.5 mL, and 20 mg/mL in a single-dose prefilled syringe.

Before starting treatment obtain baseline blood phenylalanine concentration. The recommended initial dose is 2.5 mg subcutaneous injection once weekly for 4 weeks. Increase the dosage in steps over at least 5 weeks based on tolerability to achieve a dosage of 20 mg subcutaneously once daily. See the full prescribing information for the titration regimen.

Consider increasing the dosage to a maximum of 40 mg subcutaneously once daily in patients who have been on 20 mg once daily continuously for at least 24 weeks and who have not achieved either a 20% reduction in blood phenylalanine concentration from pre-treatment baseline or a blood phenylalanine concentration less than or equal to 600 micro mol/L.

Stop treatment in patients who have not achieved at least a 20% reduction in blood phenylalanine concentration from pre-treatment baseline or a blood phenylalanine concentration less than or equal to 600 μmol/L after 16 weeks of continuous treatment with the maximum dosage of 40 mg once daily.

Reduce the dosage and/or modify dietary protein and phenylalanine intake, as needed, to maintain blood phenylalanine concentrations within a clinically acceptable range and above 30 micromol/L.

Obtain blood phenylalanine concentrations every 4 weeks until a maintenance dosage is reached. After a maintenance dosage is established, monitor blood phenylalanine concentrations. Advise patients to monitor dietary protein and phenylalanine intake, and adjust as directed by their healthcare provider.

Palynziq active ingredients

Pegvaliase-pqpz is a PEGylated phenylalanine ammonia-lyase (PAL) enzyme that converts phenylalanine to ammonia and trans-cinnamic acid. It substitutes for the deficient phenylalanine hydroxylase enzyme activity in patients with PKU and reduces blood phenylalanine concentrations.

Palynziq side effects

The most common side effects of Palynziq in clinical trials relative to placebo include:

  • Injection site reactions
  • Arthralgia (joint pain), headache
  • Generalized skin reactions lasting at least 14 days
  • Pruritus
  • Nausea, vomiting
  • Abdominal pain, diarrhea
  • Throat pain, cough
  • Tiredness

In rare instances, Palynziq can cause more serious adverse reactions. These can include:

  • Hypersensitivity reactions – likely due to the formation of antibodies to the product

Palynziq contraindications

You should not use Palynziq if you:

  • Are allergic to the active ingredient pegvaliase-pqpz
  • Are allergic to any of the other ingredients in Palynziq
  • Are younger than 18 years old

Talk to your doctor before using Palynziq if you:

  • Are taking any of the medications that could interact with Palynziq
  • Are pregnant or are planning to become pregnant – may cause fetal harm
  • Are breastfeeding or are planning to breastfeed
