What is Pepaxto?

Pepaxto is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Oncopeptides AB. It is classed as an alkylating agent and used with dexamethasone, for the treatment of adult patients with relapsed or refractory multiple myeloma (RRMM) who have already had at least four prior lines of therapy and whose disease is refractory to at least one proteasome inhibitor, one immunomodulatory agent, and one CD38-directed monoclonal antibody.

This indication is approved under accelerated approval based on data from the Horizon study, a multicenter, single-arm trial to determine the safety and efficacy of Pepaxto in patients with RRMM. Continued approval may depend upon verification and description of clinical benefits in confirmatory trials.

What doses of Pepaxto are available?

Pepaxto is available in injection form, in the following doses: 20 mg melphalan flufenamide as a lyophilized powder in a single-dose vial.

Pepaxto active ingredients

The active ingredient of Pepaxto is melphalan flufenamide (also known as melflufen).

Pepaxto side effects

The most common side effects of Pepaxto in clinical trials include:

  • Fatigue
  • Nausea
  • Diarrhea
  • Pyrexia
  • Respiratory tract infection
  • Laboratory abnormalities – decreased leukocytes, platelets, lymphocytes, neutrophils, and hemoglobin
  • Increased creatinine

In rare instances, Pepaxto can cause more serious side effects. These can include:

  • Increased risk of death at doses higher than recommended
  • Thrombocytopenia
  • Neutropenia
  • Anemia
  • Infections
  • Secondary malignancies
  • Embryo-fetal toxicity

Pepaxto drug interactions

Pepaxto contraindications

You should not use Pepaxto if you:

  • Are allergic to the active ingredient melphalan flufenamide
  • Are allergic to any of the other ingredients in Pepaxto
  • Are breastfeeding or are planning to breastfeed

  • Are taking any of the medications that could interact with Pepaxto

Pepaxto generic

Pepaxto is the trademark brand name for melphalan flufenamide manufactured by Oncopeptides Inc. A generic version of melphalan flufenamide is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Pepaxto savings through NiceRx.
