What is Recorlev?

Recorlev is an FDA (U.S. Food and Drug Administration) approved medication developed by Strongbridge Biopharma (now acquired by Xeris Pharmaceuticals, Inc.) for endogenous Cushing’s syndrome.

FDA approval of Recorlev as a safe and effective treatment option for adult patients with endogenous Cushing’s syndrome is based on results from the open-label SONICS trial that showed Recorlev reduces and causes normalization of mean urinary-free cortisol concentrations without a dose increase. Results from the LOGICS trial confirmed findings from the SONIC trial.

The FDA has approved Recorlev, (Xeris Biopharma Holdings) for the treatment of endogenous hypercortisolemia (high cortisol levels) in adult patients with Cushing’s syndrome for whom surgery has not been curative or is not an option. Recorlev is not used for the treatment of fungal infections.

What doses of Recorlev are available?

Recorlev is available in tablet form, in the following doses: 150 mg.

The boxed warnings include hepatotoxicity, hypercortisolism, hypersensitivity reaction, QT prolongation, and risks related to decreased testosterone. Please read the prescribing information provided and always speak with your healthcare provider for medical advice about any changes to your dose so they can monitor and evaluate your condition.

Recorlev active ingredients

Recorlev contains levoketoconazole a cortisol synthesis inhibitor, as the active ingredient.

Recorlev side effects

The most common side effects of Recorlev in clinical trials compared to placebo include:

  • Nausea, vomiting, stomach pain
  • Low potassium (hypokalemia)
  • Easy bleeding, easy bruising
  • High blood pressure, headache
  • Abnormal uterine bleeding
  • Feeling tired
  • Muscle and back pain
  • Problems sleeping
  • Stuffy nose, sneezing, sore throat

In rare instances, Recorlev can cause more serious side effects. These can include:

  • Faster heart rate, shortness of breath, and sudden dizziness or feeling faint
  • Decreased adrenal gland hormones
  • Breast enlargement and erectile dysfunction in men
  • Mood changes, reduced sex drive in women
  • Hepatotoxicity – upper right-sided stomach pain, loss of appetite, dark urine, clay-colored stools, yellowing of the skin or eyes

Recorlev drug interactions

Other drugs may interact with Recorlev, including prescription drugs and over-the-counter medicines, vitamins, and herbal products. Tell your caregiver about all of your medications including any supplements you are taking.

Recorlev contraindications

You should not use Recorlev if you:

  • Are allergic to the active ingredient levoketoconazole, or ketoconazole
  • Have had hypersensitivity reactions to any of the other ingredients in Recorlev
  • Are pregnant or are planning to become pregnant
  • Are breastfeeding or are planning to breastfeed – Do not breastfeed while receiving treatment, and for at least 1 day after your last dose

Talk to your doctor before using Recorlev if you:

  • Are taking any of the medications that could interact with Recorlev
  • Have liver disease
  • Have gallstones
  • Have a heart rhythm disorder
  • Have congestive heart failure
  • Have long QT syndrome
  • Have an electrolyte imbalance

Recorlev generic

Recorlev is the trademark brand name for levoketoconazole manufactured by Xeris Pharmaceuticals, Inc. A generic version of levoketoconazole is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Recorlev savings through NiceRx.
