What is Ruconest?
Ruconest is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Pharming Healthcare, Inc. Ruconest is a C1 esterase inhibitor [recombinant] used for the treatment of acute attacks in adults and adolescent patients with hereditary angioedema (HAE). It is not known if Ruconest is effective for treating HAE attacks that affect your throat.
With HAE, you have low levels of C1 esterase inhibitor (C1-INH), or you have a faulty version of it. Ruconest replaces the missing or faulty protein in your blood and helps control attacks of HAE.
What doses of Ruconest are available?
Ruconest is available as 2100 IU lyophilized powder for reconstitution for injection in a single-use vial.
Reconstitute each vial (2100 IU) by adding 14 mL sterile water for injection per vial to make a solution of 150 IU per mL. Administer the solution at room temperature as a slow intravenous injection over 5 minutes.
If the symptoms continue, a second dose can be given at the recommended dose level. Do not exceed 4200 IU per dose and no more than two doses should be administered within a 24 hour period. Your dosage will depend on your body weight in kilograms (kg).

Ruconest active ingredients
Ruconest contains the active ingredient conestat alfa, a rhC1INH protein replacement therapy.
Ruconest side effects
The most common side effects of Ruconest in clinical trials compared to placebo include:
- Headache, nausea, and diarrhea
In rare instances, Ruconest can cause more serious adverse effects. These can include:
- Hypersensitivity reactions – hives, generalized urticaria, chest tightness, wheezing, hypotension, and anaphylaxis
- Serious arterial and venous thromboembolic (TE) events
Your doctor will assess the benefits of using Ruconest against your risk of side effects. Patients are encouraged to report negative side effects or adverse reactions of Ruconest to the FDA. Ruconest drug interactions
No drug interaction studies have been conducted with Ruconest. Talk to your caregivers about all the medicines you take, including prescription drugs and over-the-counter medicines, vitamins, and herbal supplements.
Ruconest contraindications
You should not use Ruconest if you:
- Have had an allergic reaction to the active ingredient conestat alfa
- Are allergic to any of the other ingredients in Ruconest
- Have a known or suspected allergy to rabbits and rabbit-derived products
- Have a history of immediate hypersensitivity reactions, including anaphylaxis, to C1 esterase inhibitor preparations
