What is Signifor LAR?
Signifor LAR is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Recordati. Signifor LAR is a somatostatin analog that decreases the amount of growth hormone made in the body. It is used to treat:
- Patients with acromegaly who have not responded to surgery and/or for whom surgery is not an option. Treating acromegaly reduces the risk of serious problems such as diabetes and heart disease
- Patients with Cushing’s disease for whom pituitary surgery is not an option or has not been curative. Cushing’s disease occurs when the body makes too much cortisol. High cortisol levels in the body may lead to high blood pressure and high blood sugar. Pasireotide works by decreasing the amount of cortisol made in the body
What doses of Signifor LAR are available?
Signifor LAR is available as an injectable suspension, in the following doses: 10 mg, 20 mg, 30 mg, 40 mg, and 60 mg, powder in a vial for reconstitution with 2 mL diluent.
Check fasting plasma glucose, hemoglobin A1c, liver enzyme tests, electrocardiogram (ECG), serum magnesium, and serum potassium, and ensure glucose levels are well controlled in patients with poorly controlled diabetes mellitus before starting treatment.
The initial dosage for acromegaly is 40 mg once every 4 weeks (every 28 days). The initial dosage for Cushing’s Disease is 10 mg by intramuscular injection once every 4 weeks (every 28 days).
For patients with hepatic impairment, Child-Pugh B the initial recommended dose for acromegaly is 20 mg once every 4 weeks with a maximum dose of 40 mg once every 4 weeks. For patients with hepatic impairment, Child-Pugh B the initial recommended dose for Cushing’s Disease is 10 mg once every 4 weeks with a maximum dose of 20 mg once every 4 weeks. Signifor LAR is not recommended for patients with Child-Pugh C.
If you miss a dose of Signifor LAR, you can receive your missed dose up to 14 days before your next dose. Signifor LAR can increase blood sugar levels and may cause diabetes. Your blood sugar may need to be tested before and during treatment.

The most common side effects of Signifor LAR in clinical trials include:
- Cholelithiasis (gallstones) – complications include inflammation and infection of the gall bladder

- Injection site reactions
- High blood pressure
- Swelling, rapid weight gain
- Headache, back pain
- Cold symptoms
- Hair loss
- Unusual tiredness, weight loss
Signifor LAR can cause more serious side effects. These can include:
- Suppression of pituitary hormones other than GH/IGF-1
- Liver test elevations – check liver enzymes before starting treatment
- Liver disease (such as loss of appetite, abdominal pain, bloating, yellowing of eyes/skin, dark urine
- Bradycardia (slow heartbeat) and QT prolongation – check ECG and electrolytes before starting treatment
- Pituitary hormone deficiency

Signifor LAR drug interactions
Signifor LAR can interact with other medications. These include:
- Drugs that prolong QT
- Calcium channel blockers
- Potassium and magnesium electrolytes
- Bromocriptine – consider bromocriptine dose reduction

Signifor LAR contraindications
You should not use Signifor LAR if you:
- Are allergic to the active ingredient pasireotide
- Are allergic to any of the other ingredients in Signifor LAR
- Are pregnant or are planning to become pregnant – there is potential for an unintended pregnancy
Talk to your doctor before using Signifor LAR if you:
- Are taking any of the medications that could interact with Signifor LAR
- Are breastfeeding or are planning to breastfeed
