What is Sylvant?

Sylvant is an FDA (U.S. Food and Drug Administration) approved medication manufactured by EUSA Pharma. It is an interleukin-6 (IL-6) antagonist indicated for the treatment of patients with multicentric Castleman’s disease (MCD) who are human immunodeficiency virus (HIV) negative and human herpesvirus-8 (HHV-8) negative.

MCD is a rare blood disorder with high morbidity in which lymphocytes are over-produced, leading to enlarged lymph nodes. MCD can also affect lymphoid tissue of internal organs, causing the liver, spleen, or other organs to enlarge. Infections, multisystem organ failure, and malignancies including malignant lymphoma are common causes of death in patients with MCD.

What doses of Sylvant are available?

Sylvant is available in lyophilized powder form, in the following doses: 100 mg of lyophilized powder in a single-use vial, 400 mg of lyophilized powder in a single-use vial.

Sylvant is for intravenous infusion only. The recommended dose is 11 mg/kg given over 1 hour by intravenous infusion every 3 weeks.

Sylvant active ingredients

The active substance in Sylvant, siltuximab, is a monoclonal antibody.

Sylvant side effects

The most common side effects of Sylvant in clinical trials compared to placebo include:

  • Pruritus
  • Weight gain
  • Rash
  • Hyperuricemia
  • Upper respiratory tract infection

In rare instances, Sylvant can cause more serious side effects. These can include:

  • Severe infusion-related reactions, anaphylaxis
  • Severe allergic reactions
  • Gastrointestinal (GI) perforation

Sylvant drug interactions

Sylvant can interact with other medications. These include:

  • Cytochrome P450s in the liver are downregulated by infection and inflammation stimuli including cytokines such as IL-6. Inhibition of IL-6 signaling in patients treated with Sylvant may restore CYP450 activities to higher levels leading to increased metabolism of drugs that are CYP450 substrates – oral contraceptives, lovastatin, atorvastatin
  • Vaccinations – do not administer live vaccines because IL-6 inhibition may interfere with the normal immune response to new antigens

Sylvant contraindications

You should not use Sylvant if you:

  • Have had a severe hypersensitivity reaction to the active ingredient siltuximab
  • Have had an allergic reaction to any of the other ingredients in Sylvant
  • Have a severe infection
  • Are pregnant or are planning to become pregnant
  • Are breastfeeding or are planning to breastfeed

Talk to your doctor before using Sylvant if you:

  • Are taking any of the medications that could interact with Sylvant
  • Have a stomach or intestinal disorder, including ulcer or diverticulitis
  • Have gout

Sylvant generic

Sylvant is the trademark brand name for siltuximab manufactured by EUSA Pharma. A generic version of siltuximab is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Sylvant savings through NiceRx.
