What is Zavesca?

Zavesca is an FDA (U.S. Food and Drug Administration) approved medication manufactured for Actelion Pharmaceuticals US, Inc., a Janssen Pharmaceutical Company. It is a glucosylceramide synthase inhibitor (the enzyme responsible for the first step in the synthesis of most glycosphingolipids) indicated as monotherapy to treat adult patients with mild to moderate type 1 Gaucher disease for whom enzyme replacement therapy (ERT) is not an option.

Type 1 Gaucher disease is caused by a lack of glucocerebrosidase, the enzyme needed for the breakdown of glycosphingolipid glucosylceramide. Lipids build up in the body and cause symptoms such as easy bruising or bleeding, weakness, anemia, bone or joint pain, enlarged liver or spleen, or weakened bones.

Zavesca is also used in patients of all ages with Niemann-Pick type-C disease, a potentially fatal disease in which glycosphingolipids build up within cells in the brain and elsewhere in the body. Zavesca is used to treat neurological symptoms which include loss of coordination, problems with eye movements that can lead to vision problems, delayed development, problems swallowing, decreased muscle tone, seizures, and learning difficulties.

What doses of Zavesca are available?

Zavesca is available in capsule form, in the following doses: 100 mg.

The recommended dosage is a 100 mg capsule, three times a day at regular intervals. The dose may be reduced to 100 mg once or twice a day in patients due to tremors or diarrhea. Dosing for patients under the age of 12 years should be adjusted according to body surface area.

Zavesca side effects

The most common side effects of Zavesca in clinical trials (incidence ≥5%) include:

  • Constipation, weight loss, stomach pain, flatulence, nausea, and vomiting
  • Diarrhea can be serious – you may need to make changes to your diet, such as avoiding foods high in carbohydrates
  • Headache, including migraine
  • Tremor, leg cramps, dizziness, weakness
  • Vision problems
  • Thrombocytopenia (reduced platelet count)
  • Muscle cramps, back pain
  • Dry mouth
  • Memory loss, unsteady walking
  • Stomach bloating, abdominal pain not related to food, and menstrual changes

In rare instances, Zavesca can cause more serious side effects. These can include:

  • Peripheral neuropathy – perform baseline and follow-up neurological evaluations at 6-month intervals
  • Tremor or exacerbation of existing tremors

Zavesca drug interactions

Other drugs may interact with Zavesca, including prescription drugs and over-the-counter medicines, vitamins, and herbal products. Tell your health care provider about all of your medication including any supplements you are taking.

Zavesca contraindications

You should not use Zavesca if you:

  • Are allergic to the active ingredient miglustat
  • Have known hypersensitivity to any of the other ingredients in Zavesca
  • Are pregnant or are planning to become pregnant – Based on animal data, may cause fetal harm
  • Are breastfeeding or are planning to breastfeed – changes in lactation were seen in animal studies and it is likely to be present in breast milk

Talk to your doctor about your medical conditions before using Zavesca if you:

  • Are taking any of the medications that could interact with Zavesca such as imiglucerase
  • Have stomach problems
  • Have gastrointestinal disease, such as inflammatory bowel disease
  • Have kidney disease
  • Have a condition that causes tremors

Zavesca generic

Zavesca is the trademark brand name for miglustat manufactured for Actelion Pharmaceuticals US, Inc., a Janssen Pharmaceutical Company. A generic version of miglustat is available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Zavesca savings through NiceRx.
