What is Zemdri?

Zemdri is an FDA (U.S. Food and Drug Administration) approved medication developed by Achaogen. It is an aminoglycoside antibacterial used for the treatment of Complicated Urinary Tract Infections (cUTI) including Pyelonephritis in patients 18 years of age or older caused by the following susceptible microorganism(s): Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, and Enterobacter cloacae.

Zemdri should be used in patients who have limited or no alternative treatment options as only limited clinical safety and efficacy data are available. To reduce the emergence of drug-resistant bacteria and maintain the effectiveness of Zemdri and other antibacterial drugs, it should only be used to treat infections suspected to be caused by susceptible microorganisms.

What doses of Zemdri are available?

Zemdri is available in injection form, in the following doses: 500 mg/10 mL (50 mg/mL) as a single-dose vial.

Administer as an intravenous infusion over 30 minutes to patients 18 years of age or older with creatinine clearance greater than or equal to 90 mL/min. The recommended duration of treatment is 4 to 7 days for cUTI, including pyelonephritis. Adjust the dose for patients with renal impairment.

Zemdri active ingredients

Zemdri contains plazomicin sulfate, a semisynthetic aminoglycoside as the active ingredient. Made to overcome aminoglycoside-modifying enzymes, the most common aminoglycoside-resistance mechanism in Enterobacteriaceae, and has in vitro activity against extended-spectrum beta-lactamase-producing, aminoglycoside-resistant, and carbapenem-resistant isolates.

Zemdri side effects

The most common side effects of Zemdri in clinical trials include:

  • Decreased renal function
  • Diarrhea
  • Hypertension, hypotension
  • Headache
  • Nausea, and vomiting

In rare instances, Zemdri can cause more serious side effects. These can include:

  • Hypersensitivity reactions, including anaphylaxis and allergic reactions
  • Clostridium difficile (C. difficile)-associated diarrhea
  • Worsening of muscle weakness in patients with neuromuscular problems, or delay in recovery of neuromuscular function in patients receiving neuromuscular blocking agents
  • Nephrotoxicity
  • Neuromuscular blockade – monitor high-risk patients
  • Fetal harm
  • Ototoxicity – symptoms include hearing loss, tinnitus, and/or vertigo

Zemdri drug interactions

Other drugs may interact with Zemdri, including prescription drugs and over-the-counter medicines, vitamins, and herbal products. Tell your health care provider about all of your medications including any supplements you are taking.

Zemdri contraindications

You should not use Zemdri if you:

  • Are allergic to the active ingredient plazomicin
  • Are allergic to any of the other ingredients in Zemdri
  • Are pregnant or are planning to become pregnant

Talk to your doctor before using Zemdri if you:

  • Are taking any of the medications that could interact with Zemdri
  • Have kidney disease
  • Have a nerve-muscle disorder such as myasthenia gravis
  • Are breastfeeding or are planning to breastfeed

Zemdri generic

Zemdri is the trademark brand name for plazomicin manufactured by Cipla USA, Inc. A generic version of plazomicin is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Zemdri savings through NiceRx.
