What is Pemfexy?

Pemfexy (pemetrexed injection) is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Eagle Pharmaceuticals, Inc. Pemfexy is a type of antifolate used:

  • Alongside cisplatin for the initial treatment of patients with locally advanced or metastatic non-squamous non-small cell lung cancer (NSCLC)
  • As a single agent for the maintenance treatment of patients with locally advanced or metastatic non-squamous NSCLC whose disease has not progressed after four cycles of platinum-based first-line chemotherapy
  • As a single agent for the treatment of patients with metastatic non-squamous NSCLC that has returned after prior chemotherapy

Pemfexy is not recommended for the treatment of patients with squamous cell non-small cell lung cancer with cisplatin for the initial treatment of patients with malignant pleural mesothelioma whose disease is unresectable or who are otherwise not suitable for curative surgery.

What doses of Pemfexy are available?

Pemfexy is available in injection form, in the following doses: 25 mg/mL in a single-dose vial.

The recommended dose of Pemfexy as a single agent or with cisplatin, in patients with creatinine clearance of 45 mL/minute or greater, is 500 mg/m2, over ten minutes as an intravenous infusion on Day 1 of each 21-day cycle. Administer until disease progression or unacceptable toxicity.

Start folic acid 400 mcg to 1000 mcg orally once daily, 7 days before the first dose, and continue until 21 days after the last dose. Take vitamin B12, 1 mg intramuscularly 1-week before the first dose and every 3 cycles after. Also, take dexamethasone 4 mg orally twice daily, the day before, on the day, and the day after administration.

Pemfexy side effects

The most common side effects of Pemfexy in clinical trials include:

  • Tiredness, nausea, constipation, vomiting, and anorexia
  • Stomatitis/pharyngitis
  • Neutropenia
  • Anemia
  • Thrombocytopenia

In rare instances, Pemfexy can cause more serious side effects. These can include:

  • Myelosuppression
  • Renal Failure
  • Bullous and exfoliative skin toxicity
  • Interstitial pneumonitis
  • Radiation recall
  • Embryo-Fetal toxicity

Pemfexy drug interactions

Pemfexy can interact with other medications. These include:

  • Ibuprofen – modify the ibuprofen dosage for patients with a creatinine clearance between 45 mL/min and 79 mL/min

Pemfexy contraindications

You should not use Pemfexy if you:

  • Are allergic to the active ingredient pemetrexed
  • Are allergic to any of the other ingredients in Pemfexy
  • Are pregnant or are planning to become pregnant

  • Are taking any of the medications that could interact with Pemfexy
  • Are breastfeeding or are planning to breastfeed

Pemfexy is the trademark brand name for pemetrexed manufactured by Eagle Pharmaceuticals, Inc. A generic version of pemetrexed is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Pemfexy savings through NiceRx.
