What is Samsca?

Samsca contains the prescription medicine tolvaptan, which increases low sodium levels in the blood (hyponatremia) in adults with heart failure and specific hormone imbalances. Samsca does this by helping to remove water from your body as urine. This causes your body’s salt concentration to increase.

Samsca should not be used if you are dehydrated, if you can not tell you are thirsty, or if you are unable to urinate. Samsca can be dangerous if your low sodium levels are reversed too quickly. You must inform your doctor immediately if you have difficulty with your speech, swallowing, moving your arms or legs, any confusion, or seizures. You will need regular checks while you are taking this medicine.

Samsca active ingredients

Tolvaptan blocks certain receptors in your body known as vasopressin V2-receptors. When taken tolvaptan blocks the effects of vasopressin in the body. This causes you to get rid of more water in your urine and has the effect of raising your serum sodium concentrations. Tolvaptan does not increase the amount of sodium lost in your urine.

You should take Samsca with plenty of water once per day. When you start or if you restart this medicine, you will need to be observed in a hospital just in case you have any serious side effects. Samsca should only be used for up to 30 days as long-term use can cause liver damage.

Samsca side effects

The most common side effects caused by Samsca include:

  • Dry mouth
  • Increased thirst or urination
  • Hunger
  • Constipation
  • Feeling tired or weak

In rare instances, Samsca can cause more serious side effects. These can include:

  • Weakness, confusion, light-headed
  • Irregular heartbeats
  • Seizures
  • Weight loss
  • Increased thirst, difficulty urinating, heavy sweating, hot and dry skin
  • Upper right side stomach pain, vomiting, loss of appetite, fever, itching, yellow color to your skin or eyes
  • Problems with speech or muscle control, difficulty moving your arms and legs
  • Difficulty swallowing
  • Confusion
  • Mood changes

Samsca drug interactions

Samsca can interact with other medications. These include:

  • Antiviral treatments – amprenavir, atazanavir, boceprevir
  • Anticonvulsants to prevent seizures – carbamazepine, phenobarbital, phenytoin
  • Antibiotics for bacterial infections – clarithromycin, rifampin, telithromycin
  • Antifungal treatments – itraconazole, ketoconazole, voriconazole
  • DMARDs (disease-modifying antirheumatic drugs) for rheumatoid arthritis – teriflunomide
  • Treatments for heart arrhythmias (irregular heartbeat) – amiodarone, dronedarone
  • Blood thinners – apixaban, edoxaban, heparin

This list is not exhaustive and other drugs may interact with Samsca.

Samsca warning and precautions

You should not use Samsca if you:

  • Are allergic to the active ingredient tolvaptan
  • Are allergic to any of the other ingredients in Samsca
  • Are unable to urinate
  • Are unable to tell that you are thirsty
  • Are severely dehydrated and feel like you might pass out
  • Need your sodium levels to be raised quickly
  • Are breastfeeding or are planning to breastfeed

Talk to your doctor before using Samsca if you:

  • Are taking any of the medications that could interact with Samsca
  • Have kidney disease or urination problems
  • Have liver problems
  • Drink large amounts of alcohol
  • Are malnourished
  • Use desmopressin or a water pill
  • Are pregnant or are planning to become pregnant
