What is Besremi?

Besremi is an FDA (U.S. Food and Drug Administration) approved medication manufactured by PharmaEssentia Corporation. FDA approval is based on the results of the Peginvera study, a prospective, multicenter, single-arm trial of 7.5 years duration.

Besremi is an interferon alfa-2b used for the treatment of adults with polycythemia vera. Polycythemia vera is a blood disease that results from the overproduction of red blood cells due to genetic mutations involving the Janus Kinase 2 (JAK2) gene. Patients with polycythemia vera have blood that is thicker than normal, potentially resulting in increased risks for thrombosis and reduction in blood flow.

Risk of serious disorders – Interferon alfa products may cause or aggravate fatal or life-threatening neuropsychiatric, autoimmune, ischemic, and infectious disorders. Patients should be monitored closely and therapy withdrawn with persistently severe or worsening signs or symptoms of the above disorders.

What doses of Besremi are available?

Besremi is available in injection form, in the following doses: 500 mcg/mL solution in a single-dose prefilled syringe.

The recommended starting dose is 100 mcg by subcutaneous injection every 2 weeks (50 mcg if receiving hydroxyurea). Increase the dose by 50 mcg every 2 weeks (up to a maximum of 500 mcg) until stabilized.

Besremi active ingredients

The active ingredient of Besremi is ropeginterferon alfa-2b-njft, a monopegylated, long-acting interferon.

Besremi is thought to work by attaching to specific receptors in the body, setting off a chain reaction that makes the bone marrow reduce blood cell production.

The effectiveness and safety of Besremi were evaluated in a multicenter, single-arm trial that lasted 7.5 years. Besremi’s effectiveness was assessed by looking at how many patients achieved a complete hematological response. This meant that patients had a red blood cell volume of less than 45% without a recent phlebotomy, normal white cell counts and platelet counts, a normal spleen size, and no blood clots. Overall, 61% of patients had a complete hematological response.

Besremi side effects

The most common side effects of Besremi in clinical trials include:

  • Influenza-like illness
  • Arthralgia, musculoskeletal pain, fatigue
  • Pruritus
  • Nasopharyngitis
  • Low levels of white blood cells and platelets

In rare instances, Besremi can cause more serious side effects. These can include:

  • Depression and suicide
  • Cardiovascular toxicity
  • Decreased peripheral blood counts
  • Hypersensitivity reactions
  • Pancreatitis
  • Colitis
  • Pulmonary toxicity
  • Ophthalmologic toxicity
  • Hyperlipidemia
  • Hepatotoxicity – monitor liver enzymes and hepatic function at baseline and during treatment
  • Renal toxicity
  • Dental and periodontal toxicity
  • Dermatologic toxicity
  • Driving and operating machinery – avoid driving or using machinery if you experience dizziness, somnolence, or hallucination
  • Urinary tract infections

Besremi drug interactions

Besremi can interact with other medications. These include:

  • CYP450 substrates
  • Myelosuppressive agents
  • Narcotics, hypnotics, or sedatives

This list is not exhaustive and other prescription drugs may interact with Besremi.

Besremi contraindications

You should not use Besremi if you:

  • Are allergic to the active ingredient ropeginterferon alfa-2b-njft
  • Are allergic to any of the other ingredients in Besremi

Talk to your doctor before using Besremi if you:

  • Are taking any of the medications that could interact with Besremi
  • Have a history of severe psychiatric disorders
  • Have hypersensitivity to interferon
  • Have hepatic impairment
  • Have a history or presence of active serious or untreated autoimmune disease
  • Are pregnant or are planning to become pregnant
  • Are breastfeeding or are planning to breastfeed

Besremi generic

Besremi is the trademark brand name for ropeginterferon alfa-2b njft manufactured by PharmaEssentia Corporation. A generic version of ropeginterferon alfa-2b njft is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Besremi savings through NiceRx.
