What is Janumet XR?

Janumet XR is a prescription medicine given to adults with type 2 diabetes, a condition where you have high blood sugar levels for prolonged periods. It’s used to help lower blood sugar levels and should be taken in line with changes to diet and exercise. You should take a Janumet XR pill orally once a day. 

Your digestive system releases sugar into your blood from the food you eat. These sugars are needed by your cells – they use a hormone called insulin to absorb the sugar from your blood. People who have type 2 diabetes become resistant to insulin, or simply don’t produce enough of it. When either of these things happens, it means that less sugar is absorbed from the blood causing blood sugar levels to remain too high. 

In the short-term, high blood sugar levels can make you feel hungry and thirsty, as well as causing you to urinate more regularly. Over a longer period, high blood sugar levels can lead to serious health problems, such as heart diseases, kidney failure, strokes, and nerve damage. 

Janumet XR helps to lower your blood sugar levels. It does this in two distinct ways. Firstly, by encouraging your pancreas to continue producing insulin for longer following food which helps your body to absorb more sugar. Secondly, it causes your liver to release less sugar into your blood during the digestive process after you’ve eaten. These two processes combined help to maintain lower blood sugar levels in your body. 

Janumet XR active ingredients

There are two active ingredients within Janumet XR. Sitagliptin and metformin – they each enable Janumet XR to conduct its dual processes within your body. By combining these two active ingredients and processes, Janumet XR helps lower your blood sugar levels.

After you eat your body digests your food. One aspect of this is your stomach and gut producing digestive hormones that cause your pancreas to secrete insulin. Over time, these digestive hormones are normally broken down. Sitagliptin slows this, ensuring that your digestive enzymes are active for longer. This in turn enables you to produce more insulin. The process of regulating how much sugar is released into your blood by your liver is controlled by metformin.

Janumet XR side effects & Janumet XR drug interactions

The most common side effects caused by Janumet XR include:

  • A blocked or runny nose
  • A sore throat
  • Headaches
  • Diarrhea
  • Nausea
  • Vomiting
  • Indigestion and gas
  • An increase in upper respiratory tract infections (throat, nose, sinuses)
  • Hypoglycemia – your blood sugar falling too low – particularly when used with other medications like insulin or sulfonylurea

Sometimes, people can suffer from more serious Janumet XR side effects. These are rarer, but can include: 

  • Severe allergic reactions 
  • Lactic acidosis – a buildup of lactic acid in your body that can cause breathing difficulties, malaise, sleepiness, muscle pain, and stomach pain
  • Heart failure
  • Severe hypoglycemia
  • Severe joint pain (arthralgia)
  • Severe skin reactions that can cause blistering and the loss of skin
  • Inflammation of the pancreas (pancreatitis)
  • Kidney problems, including kidney failure

Janumet XR can interact with other medications, including: 

  • Carbonic anhydrase inhibitors (acetazolamide, zonisamide, or dichlorphenamide)
  • Cationic drugs (vancomycin, amiloride, quinidine, quinine, trimethoprim, digoxin, morphine, triamterene, procainamide, or ranitidine)
  • Medications that can lower your blood sugar levels (nicotinic acid, diuretics, oral contraceptives, calcium channel blocking drugs, corticosteroids, and estrogens)

Tell your prescribing physician if you are taking any of the above, as when taken with Janumet XR they can potentially alter how it works, making it less effective and causing certain side effects to be worse. 

Janumet XR warnings & precautions

Janumet XR isn’t suitable for everyone. You shouldn’t take it if you are allergic to either of the active ingredients (sitagliptin or metformin), or any of the other ingredients found in the medication. You can find these listed in the leaflet that comes with your medication. 

Don’t take Janumet XR if you:

  • Have type 1 diabetes 
  • Have diabetic ketoacidosis. This condition is caused by diabetes when body fat is broken down too quickly. It can potentially be life-threatening. 
  • Have severe kidney problems

If you are under 18 years of age, you should not take Janumet XR. You should consult your physician if you are over 65 years of age. 

Talk to your doctor before taking Janumet XR if you are pregnant or breastfeeding or trying to get pregnant and planning to breastfeed. It’s important to let your physician know if you have any of the following: 

  • A history of pancreatitis
  • Any heart problems (congestive heart failure), liver problems, or kidney problems
  • Stones in your gallbladder (gallstones)
  • High blood triglyceride levels

You should also let them know if you:

  • Are going to get an injection of dye or contrast agents for an x-ray
  • Drink alcohol often or binge drink alcohol. Try and avoid drinking alcohol when you take Janumet XR
