What is Keytruda?

Keytruda is a medicine used to treat a range of cancers. You may be prescribed Keytruda if you have:

• A skin cancer called melanoma
• A skin cancer called Merkel cell carcinoma (MCC)
• A lung cancer called non-small cell lung cancer (NSCLC)
• A lung cancer called small cell lung cancer (SCLC)
• Head and neck squamous cell cancer (HNSCC)
• Classical Hodgkin lymphoma (cHL)
• Mediastinal B-cell lymphoma (PMBCL)
• A bladder and urinary tract cancer called urothelial carcinoma
• A cancer called a microsatellite instability-high (MSI-H) or a mismatch repair deficient (dMMR) solid tumor
• A stomach cancer called gastric or gastroesophageal junction (GEJ) adenocarcinoma
• A cancer called squamous cell carcinoma of the esophagus
• Cervical cancer
• A liver cancer called hepatocellular carcinoma
• A kidney cancer called renal cell carcinoma (RCC)
• A uterine cancer called endometrial carcinoma

Keytruda is often used when your cancer has spread, if it can’t be removed by surgery, it hasn’t responded to chemotherapy, and/or if your cancer tumors have certain abnormal genes. Rather than being a type of chemotherapy or radiotherapy, Keytruda is an immunotherapy that works with your immune system to help you fight your cancer. It’s given through a drip attached to one of your veins (intravenous), usually under the supervision of a doctor or nurse.

Cancer is a group of diseases that cause abnormal cell growths called tumors. The cells in your body can divide and grow. Usually, this is kept under control by your body, older cells are destroyed and replaced by new ones. However, mistakes in the DNA code inside cells can sometimes cause them to keep dividing and growing when they aren’t supposed to. This causes a tumor.

Cancerous tumors can keep growing, and they spread around your body, causing disruption and damage. They’re often treated using chemotherapy or radiotherapy to kill the tumors. But in some cases, these treatments may not be effective or may not be suitable for your cancer. In these instances, Keytruda may be a better option. It can be used alone, or it can be used alongside other treatments like chemotherapy or radiotherapy.

Keytruda active ingredients

The active ingredient in Keytruda is called pembrolizumab. It works by helping your immune system to fight cancerous tumors.

Some cancerous tumors protect themselves from your immune system. They produce a protein called programmed cell death protein 1 (PD-1). This protein forms on the outside of the tumor and it turns your immune system cells off. This stops your immune system from attacking and destroying the tumor. But pembrolizumab removes this protection. It attaches itself to the PD-1 proteins and stops them from working. This allows your immune system to fight the cancerous tumor.

Keytruda side effects & Keytruda drug interactions

The most common side effects caused by Keytruda include:

• Headaches
• A cough
• Shortness of breath
• Stomach pain
• Diarrhea
• Constipation
• Nausea
• Vomiting
• A fall in your red blood cells
• A fall in your white blood cells
• Reduced thyroid activity
• Feeling less hungry
• Feeling tired and weak
• Dizziness
• Itching
• A skin rash
• Muscle pain
• Pain in your bones
• Joint pain

In rare instances, Keytruda can cause more serious side effects, including:

• Severe allergic reactions to the medication
• Severe infusion reactions,
• Rejection of a transplanted organ

Keytruda fights cancer by helping your immune system to target cancerous tumors. It can sometimes cause your immune system to attack normal organs and tissue as well. This can damage your organs and tissue, and if this damage becomes severe, it may be life-threatening. 

You should make yourself aware of the symptoms of organ and tissue damage so you can seek medical attention if you develop them. They include:

• Lung problems (pneumonitis) – chest pain, shortness of breath, a new or worse cough
• Intestinal problems (colitis) – diarrhea or more bowel movements than usual, stomach tenderness and pain, stools that are black, sticky, or have blood or mucus in them
• Liver problems (hepatitis) – pain on the right side of your stomach, a yellowing of your skin or the whites of your eyes, dark urine, bruising or bleeding more easily than normal
• Kidney problems – changes in the amount or color of your urine
• Hormone gland problems (particularly the thyroid, pituitary, adrenal glands, and pancreas) – weight loss or weight gain, feeling more hungry or thirsty, increased sweating, a rapid heartbeat, feeling cold, hair loss, feeling very weak, dizziness or fainting, headaches that will not go away
• Skin problems – itching, rashes, peeling, sores, and ulcers in your mouth, nose, throat, or genital area

You may also have problems with other organs and tissues, including:

• Changes in eyesight
• Severe muscle weakness
• Severe or persistent muscle or joint pains
• Swollen lymph nodes 

Your doctor will assess your risk of side effects against the benefits of taking Keytruda. 

Keytruda can interact with other medications. When this happens Keytruda and the other medications may not be as effective. Certain side effects can also become more likely. Tell your prescribing physician about all drugs you’re taking, including vitamins and dietary supplements, particularly any medications that affect your immune system, like corticosteroids.

Keytruda warnings & precautions

Don’t take Keytruda if you:

• Are allergic to the active ingredient pembrolizumab
• Are allergic to any of the other ingredients in Keytruda (these will be listed in the leaflet which comes with the medication)

Talk to your doctor before taking Keytruda if you:

• Are taking any of the medications that could affect your immune system and interact with Keytruda
• Have an autoimmune disease, like Crohn’s disease, ulcerative colitis, or lupus
• Have pneumonia or inflammation of your lungs (pneumonitis)
• Were previously given a medication called ipilimumab and you experienced serious side effects
• Have had an allergic reaction to other monoclonal antibody therapies
• Have had any liver problems or have liver damage
• Have had any kidney problems or have kidney damage
• Have HIV or AIDS
• Have had an organ transplant
• Have had a bone marrow transplant that used donor stem cells
• Are pregnant or are planning to get pregnant
• Are breastfeeding or are planning to breastfeed

You should seek medical attention after taking Keytruda if you:

• Show signs of a serious allergic reaction, like itching, a skin rash, shortness of breath, dizziness and fainting, sweating, a fast pulse, swelling of your face, throat, and other parts of your body like your hands and feet 
• Show signs of organ or tissue damage (see above)
