What is Ocrevus?

Ocrevus is a prescription medication used to treat multiple sclerosis in adults. It is taken once every six months by an infusion directly into a vein (intravenous infusion) in your arm. 

Multiple sclerosis (MS) is a long-lasting disease that damages the nerve cells in your brain, spinal cord, and in your eyes. For some people, MS is caused by your immune system mistakenly attacking and destroying the protective coating around your nerves. Your nerve cells are covered by a protective and insulating layer of a fatty material called myelin. If your immune system destroys this myelin, your nerves can have problems sending signals through your body, and they can become inflamed and damaged. This can cause problems with muscle control, balance, vision, and other basic body functions. What causes your immune system to attack the myelin around your nerves isn’t fully understood. It’s thought to be a mixture of your genetics predisposing you to MS, and incidents in your life, like viral infections, potentially triggering it.

Specific symptoms of MS will vary but can include trouble walking, problems with coordination, muscle weakness and spasms, numbness, tingling and pain, blurred and double vision, fatigue, depression, and problems focusing and paying attention. MS causes some form of permanent disability in most people who have it.

Roughly 400,000 people are living with MS in the US, and it’s around twice as common in women than men. MS can develop at any age, but most people see the first symptoms of the disease between the ages of 20 and 40. MS isn’t curable and can vary in severity. For some people, MS will continue to get worse over time, called primary progressive MS (PPMS). Other people will have attacks of symptoms (called flare-up or relapses), but then have periods of recovery with mild or no symptoms (called remission). This is called relapsing-remitting MS (RRMS).

Although it’s incurable, medications like Ocrevus can help manage MS. Ocrevus can kill some of the white blood cells created by your immune system that are thought to damage your nerves. This can help slow the progression of PPMS and reduce the number of relapses you have with RRMS.

Ocrevus active ingredients

The active ingredient in Ocrevus is called ocrelizumab. Ocrelizumab helps protect your nerves from damage by your immune system. It targets and kills a type of white blood cell (lymphocyte) called a B cell, that’s released by your immune system. 

It’s thought that B cells trigger other immune system cells (like T cells) to destroy the protective myelin around your nerves. B cells may also cause inflammation that damages your nerves. Ocrelizumab has been designed to target a specific type of protein that some B cells have on their surface called CD20. It attaches to CD20 and starts a process that kills the B cell. This can limit the damage caused by MS and reduce the number of relapses you have. Because ocrelizumab only kills CD20 B cells, other types of B cells are still active to help your immune system fight infections. 

Ocrevus side effects & Ocrevus drug interactions

As with all medications, Ocrevus can have side effects. Not everyone will experience the same side effects though if any at all. The most common side effects caused by Ocrevus include:

• An increase in infections (Ocrevus is an immunosuppressant medication that inhibits part of your immune system), like respiratory tract infections, skin infections, and herpes infections
• Reactions at the site of injection, like redness, itching, soreness, inflammation, and pain

In rare instances, Ocrevus can cause more serious side effects, including:

• Severe allergic reactions to the medication
• Serious infusion reactions (a reaction that happens during or up to 24 hours after infusion of Ocrevus). These reactions can include fever, trouble breathing, swelling of your throat, shortness of breath, and a fast heart rate 
• An increased risk of serious infections (Ocrevus is an immunosuppressant medication that inhibits part of your immune system), including a brain infection called progressive multifocal leukoencephalopathy (PML) that can be debilitating and fatal
• Reactivation of a hepatitis B infection if you’ve previously had hepatitis B
• An increased risk of some cancers, including breast cancer

Your physician will assess your risk of side effects against the benefits of taking Ocrevus. 

Ocrevus can interact with other medications. When taken together this can change how Ocrevus and the other medications work. In some instances, side effects may become more likely. Tell your prescribing physician about all drugs you’re taking, particularly any medications that affect your immune system, or other treatments for MS.

Ocrevus warnings & precautions

Don’t take Ocrevus if you:

• Are allergic to the active ingredient ocrelizumab
• Are allergic to any of the other ingredients in Ocrevus (these will be listed in the leaflet which comes with the medication)
• Have an active hepatitis B infection
• Are under 18 years of age

Talk to your doctor before taking Ocrevus if you:

• Are taking any of the medications that could interact with Ocrevus, like any medications that affect your immune system or other treatments for MS
• Have, or think you have a current infection
• Have ever had hepatitis B or are a carrier of the hepatitis B virus
• Have recently had a vaccination or are planning to have a vaccination
• Are pregnant or are planning to get pregnant
• Are breastfeeding or are planning to breastfeed
