What is Galafold?

Galafold is an FDA (U.S. Food and Drug Administration) approved medication developed by Amicus Therapeutics. Galafold is an alpha-galactosidase A (alpha-Gal A) pharmacological chaperone used for the treatment of adults with a confirmed diagnosis of Fabry disease and an amenable galactosidase alpha gene (GLA) variant based on in vitro assay data (an amenable mutation is a genetic change or variant that a person may have that is responsive to treatment).

This indication is approved under accelerated approval based on reduction in kidney interstitial capillary cell globotriaosylceramide substrate. Continued approval for this indication may depend upon verification and description of clinical benefit in confirmatory trials.

Mutations in the GLA gene are what cause Fabry disease. These mutations lead to an enzyme called alpha-galactosidase A becoming non-functional or only partially functional. As a result, fatty substances called globotriaosylceramide and lyso-Gb3 accumulate inside cells, which the enzyme would normally break down. This buildup damages tissues in the body, leading to progressive and irreversible organ damage.

What doses of Galafold are available?

Galafold is available in capsule form, in the following doses: 123 mg.

The recommended dosage is 123 mg orally once every other day at the same time of day. Take on an empty stomach. Do not consume food at least 2 hours before and 2 hours after taking Galafold, to give a minimum 4 hours fast. You may drink clear liquids during this 4-hour time when you cannot eat. Do not take Galafold on 2 consecutive days. If a dose is missed for the day, take the missed dose within 12 hours of the normal time that the dose should have been taken. If more than 12 hours have passed, take Galafold at the next planned dosing day and time and according to the every-other-day dosing schedule. Swallow capsules whole, do not cut, crush, or chew.

The most common side effects of Galafold in clinical trials compared to placebo include:

  • Headache
  • Nasopharyngitis (blocked or runny nose)
  • Sore throat
  • Urinary tract infection
  • Nausea
  • Pyrexia

Galafold drug interactions

Galafold can interact with other medications. These include:

  • Agalsidase, an enzyme replacement therapy for treating Fabry disease. This combination will increase the amount of agalsidase in the body, causing side effects and reduced effectiveness of the migalastat and potential worsening of the disease

This list is not exhaustive and other prescription drugs may interact with Galafold. Tell your healthcare provider about all the medicines you take, including prescription drugs and over-the-counter medicines, vitamins, and herbal supplements.

Galafold contraindications

You should not use Galafold if you:

  • Are allergic to the active ingredient migalastat
  • Are allergic to any of the other ingredients in Galafold
  • Have a severe renal impairment

Talk to your doctor before using Galafold about all of your medical conditions, or if you:

  • Are taking any of the medications that could interact with Galafold
  • Have kidney problems
  • Are pregnant or are planning to become pregnant
  • Are breastfeeding or are planning to breastfeed – Galafold may pass into breast milk

Galafold generic

Galafold is the trademark brand name for migalastat developed by Amicus Therapeutics. A generic version of migalastat is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Galafold savings through NiceRx.
