What is Nerlynx?
Nerlynx is an FDA (U.S. Food and Drug Administration) approved medication manufactured by Puma Biotechnology, Inc. Nerlynx is a tyrosine kinase inhibitor used in combination with capecitabine for the extended adjuvant treatment of adult patients with early-stage HER2-positive breast cancer, to follow adjuvant trastuzumab-based therapy.
What doses of Nerlynx are available?
Nerlynx is available in tablet form, in the following doses: 40 mg.
For antidiarrheal prophylaxis start loperamide with the first dose of Nerlynx and continue during the first 2 cycles (56 days) of treatment. Patients are advised to maintain 1-2 bowel movements per day and how to use antidiarrheal treatment regimens.
The recommended dose for Nerlynx use is 240 mg (6 tablets) given orally once daily with food, continuously for one year. Interruptions of dose and/or reductions of dose are recommended based on individual safety and tolerability. Reduce the starting dose to 80 mg in patients with severe hepatic impairment.

Nerlynx active ingredients
Neratinib is an intracellular kinase inhibitor that irreversibly binds to epidermal growth factor receptor (EGFR), human epidermal growth factor receptor 2 (HER2), and HER4.
Neratinibs mechanism of action is to reduce EGFR and HER2 autophosphorylation, MAPK, and AKT signaling pathways and shows antitumor activity in EGFR and/or HER2 expressing cancer cell lines.
Nerlynx side effects
The most common side effects of Nerlynx in clinical trials include:
- Diarrhea, dyspepsia, abdominal pain, decreased appetite, weight loss
- Nausea, vomiting
- Rash, dry skin, nail disorder, stomatitis
- Muscle spasms
- Urinary tract infection
In rare instances, Nerlynx can cause more serious side effects. These can include:
- Severe Diarrhea
- Hepatotoxicity (liver problems) – monitor liver function

Nerlynx drug interactions
Nerlynx can interact with other medications. These include:
- Avoid use with proton pump inhibitors (PPI) and H2-receptor antagonists – give Nerlynx 3 hours after the antacid
- Strong or moderate CYP3A4 inhibitors
- Strong or moderate CYP3A4 inducers – rifampin
- The minimal concentration change of P-gp substrates may lead to serious adverse reactions when used with Nerlynx
- Grapefruit, grapefruit juice, pomegranate, or pomegranate juice can increase the effects of Nerlynx
This list is not exhaustive and other prescription drugs may interact with Nerlynx. Talk to your healthcare advisor about all the medicines you take, including prescription drugs and over-the-counter medicines, vitamins, and herbal supplements.
You should not use Nerlynx if you:
- Are allergic to the active ingredient neratinib
- Are allergic to any of the other ingredients in Nerlynx
- Are pregnant or are planning to become pregnant – Embryo-Fetal Toxicity – you should use effective birth control (contraception) during treatment and for at least 1 month after your last dose of Nerlynx
Talk to your doctor before using Nerlynx if you:
- Are taking any of the medications that could interact with Nerlynx
- Are breastfeeding or are planning to breastfeed
Nerlynx is the trademark brand name for neratinib manufactured by Puma Biotechnology, Inc. A generic version of neratinib is not available. Generic drugs are generally cheaper than brand-name drugs, but you can still find Nerlynx savings through NiceRx.
